Make the sign visible before entering digits

Look at the paper under even light, then compare the original camera file rather than a messaging-app thumbnail. Follow the table line through adjacent cells: a true sign stays with the number, while a printed rule continues across the row. Check whether the clinic consistently writes explicit plus signs elsewhere. These observations can expose a poor image, but they cannot settle genuinely ambiguous handwriting.

Preserve the Sign Through Checkout

After entry, open the order summary and confirm that the sign remains visible beside the same eye and field. This catches skipped signs, eye swaps, and values copied from an adjacent field without asking either reader to interpret the prescription.
