A distance prescription is intended to correct far vision; a reading or near prescription is intended for a specified close working range. The numbers can look similar, but they are not interchangeable labels for the same pair. Before ordering, determine whether the prescriber supplied a distance correction, an ADD to be combined with it, a complete near prescription, or separate instructions for more than one pair.
Do not select “distance” merely because an online form opens on that option. Do not treat every positive sphere as a reading prescription. The intended range comes from the prescription and prescriber’s instructions, not from the sign of one number.
The intended viewing range is the first distinction
Single-vision lenses provide one optical correction across the lens. That correction may be issued for distance, near, or an intermediate task. The National Eye Institute notes that single-vision prescription lenses correct near or distance vision, but not both. Multifocal designs allocate more than one viewing range in the same lens.
For a near pair, “reading” also needs a real distance. A paperback held at 16 inches, a laptop at 24 inches, and a workbench at 30 inches are not the same optical task. A prescription may be written for a conventional reading distance or customized for a job. If the document says near, SVN, computer, intermediate, or gives a working distance, preserve that instruction.
Distance correction is not defined as “the pair worn all day.” Some people use distance glasses only for driving or television. Some wear them continuously. Similarly, a near prescription is not automatically suitable for every close task. The correct label describes the range the lenses were prescribed to serve.
Ask three questions before entering values:
- What is the farthest task this pair must make clear?
- What is the nearest task?
- Is the pair intended for stationary close work, general distance use, or a lens design that combines ranges?
The answers should agree with the prescription. If they do not, stop and ask the prescriber which document or row applies.
How sphere values can change when near power is written directly
Some prescriptions show a distance sphere and a separate ADD. Others write a complete near sphere value. In a simple case, the near sphere may reflect the distance sphere plus the near addition. That is why the near row can look more positive—or less negative—than the distance row.
For illustration only, a distance sphere of -1.00 with an ADD of +2.00 may correspond mathematically to a near sphere of +1.00 before other instructions are considered. This example does not authorize a consumer to create a near prescription. Cylinder, axis, prism, unequal ADD values, task distance, and the prescriber’s chosen design can all affect how an order should be written.
A positive near sphere also does not mean the person is simply farsighted. It may represent the combined power for a specific close task. Conversely, a near prescription can remain negative for a person with more minus distance power. Sign alone cannot identify the intended range.
If the document contains separate DV and NV rows, keep each row intact. Do not take the sphere from the near row and cylinder from the distance row unless the prescriber or lab explicitly confirms that representation. A near row may repeat cylinder and axis, omit them under a documented convention, or include different instructions.
ADD notation versus a separate near prescription
ADD is additional plus power used for near or intermediate vision in a multifocal or occupational design, or to derive a separate near correction when professionally specified. It is not a standalone replacement for sphere. Entering ADD +2.00 as SPH +2.00 discards the distance correction.
An ADD-based prescription usually keeps the distance values in the main OD and OS rows and lists ADD in its own field. The retailer then needs to know the lens design: progressive, bifocal, occupational, or another supported configuration. A separate single-vision near prescription may instead show the final near powers and require no ADD entry for that pair.
Look for terms such as:
SVDorDVfor single-vision distance;SVN,NV,near, orreadingfor near use;ADDfor additional near power;intermediate,computer, or a stated working distance for a task-specific range.
These abbreviations are clues, not a universal layout. If the form requests ADD but the prescription supplies only a complete near row, ask the retailer how that prescription should be submitted. If the document contains an ADD but the order is for single-vision distance, do not force the ADD into the sphere fields.
Order forms that assume distance correction
Many online prescription forms are built around a distance prescription. They may ask for SPH, CYL, AXIS, PD, then offer an ADD field when a multifocal lens is selected. A customer ordering a dedicated reading or computer pair can encounter a mismatch between the document and the form.
Send the full document when the form defaults to distance
Resolve the mismatch before checkout. Send the complete prescription and state the intended pair: “single-vision near at the prescribed working range,” for example. Ask whether the retailer accepts a direct near prescription or needs the prescriber to provide distance plus ADD. The answer may depend on the retailer’s lab and lens options.
Do not choose a progressive lens simply because the form accepts ADD. Do not choose single-vision distance because it is the default. Product selection should follow the optical instruction and task, not the easiest form path.

Label the finished pair by task, not by vague strength
When more than one single-vision pair is used, label the case with the task and date: distance, reading—16 in, or computer—24 in. “Strong” and “weak” are poor identifiers because the apparent power depends on the starting prescription and the intended distance.
Test a new near pair while seated at the prescribed task. Confirm the reading material, posture, and lighting before judging it. Remove near-only lenses before walking, driving, or operating machinery unless an eye-care professional has specifically prescribed and explained a design suitable for those activities.
If a pair arrives and distance is unexpectedly blurred, check the order record before assuming adaptation will solve it. Confirm whether the submitted values came from the distance row, near row, or an ADD calculation. An optician can measure the finished lenses and compare them with the order.
The central decision is not which prescription has “bigger numbers.” It is which range the prescriber intended those numbers to serve. Preserve that range from document to order to labeled case.
Measure the task before judging the pair
Working distance should be measured from the eyes to the task in the posture actually used. A book held in the lap, a phone near the chest, a sewing-machine needle, and a desktop monitor can all be called “near,” yet they occupy different distances. Measure several normal repetitions instead of stretching the arms to produce an ideal number.
Share the task and range with the prescriber; do not convert the measurement into lens power yourself.
The task also has depth. Reading a page may keep nearly every important detail in one plane. Cooking requires a recipe nearby, food on the counter, controls farther away, and movement around the room. A single-vision near pair can be excellent for the page but inconvenient or unsafe for the multi-distance task. That is a design discussion, not a reason to alter the prescribed values at checkout.
Build a document map before comparing powers
When comparing two prescriptions, create a document map before looking at the numbers. Note the issue date, distance, near, computer, SVD/SVN, and whether an ADD is shown. Then trace one eye across the entire row. A separate near row may already contain final near sphere powers; a distance row plus ADD may require the lab to use the ADD according to the selected lens design.
Applying the ADD twice or omitting it can both result from guessing how the form works.
If a retailer offers a generic “reader strength” menu, do not assume it accepts an individualized reading prescription. Ready-made readers typically do not reproduce unequal right and left sphere, astigmatism correction, prism, or measured placement. Ask whether the order route uses the complete prescription and how direct near values should be submitted.
Describe a delivered problem precisely
After delivery, judge the pair at the task for which it was prescribed, but document more than “blurry.” Record the actual distance, which eye seems affected if that can be determined without covering during a hazardous activity, whether the blur is central or peripheral, and whether the frame is slipping.
Those observations help the prescriber or dispenser separate a range mismatch, fit issue, entry error, or clinical concern without inviting self-adjustment of the prescription.
